IMPORTANT - SURVEY INSTRUCTIONS
Step 1: Enter your first name, last name, middle initial, and employee or student ID number, found on your Chapman ID card.
Step 2: Enter your home address (remains confidential), city, zip code, and cross-streets.
Step 3: Enter your 10-digit phone number (select type) and your Chapman email address.
Step 4: Enter your Work Department, select your Commute Mode used during the week of Monday, September 25, 2023 through Friday, September 29, 2023, and choose your Work Hours - Start and End Times.
Step 5: Scroll to select your mode of transportation and time you began work (AM or PM) for each day of the survey week.
Important: All five (5) days must show a commute mode, and a time you began work, even if you took a day off, were sick, or had a regular day off. For any Day Off, regardless of reason, enter the same start time you entered for days you commuted to work. This is for processing purposes only and will not affect the accuracy of the survey.
- If you took a vacation day, use mode "AA."
- If you were sick, use "BB."
- If you had a regular day off during the week (other than vacation or sick day), use "CC" as your commute mode for that day.1
- If you "worked from home," select "F" for Telecommuting
- Please do not select "Noncommuting" (mode G) unless you were outside all four counties of the SCAQMD jurisdiction: Orange, Los Angeles, Riverside, and San Bernardino.
Step 6: Commuter information: Select "Yes" or "No" as to whether you would like to receive a RideGuide (for possible ridesharing).
Submit: Click on "Submit Form."
