What is the International Supplement Form?
The International Supplement Form is a form that you must submit to begin the immigration process for studying in the United States.
The International Supplement Form lets us know:
- Your country of citizenship.
- Your current visa status.
- Your sponsorship and financial information (if you will need an F-1/J-1 visa).
When do I submit my International Supplement Form?
You will submit your International Supplement Form after you have been admitted to Chapman.
How do I submit my International Supplement Form?
After you have been admitted to Chapman, find your International Supplement Form in your Application Status Page. There, you can fill out the form and submit it.
If you applied to the program using a CAS application and do not have a login to the Application Status Page, then use this version of the International Supplement Form instead. Then submit the completed form and all additional documents to Graduate Admission Counselor, Shannon Morris Alvarez at shmorris@chapman.edu.
What documents do I need to submit with the form?
You will be asked to submit the following documents as part of your International Supplement Form:
- Photocopy of passport data and signature page.
- Evidence of available funds. These may be personal funds, family funds, funding from other sources, or loan certificates/letters. Your available funds must be equal to or greater than Chapman’s cost of attendance for graduate students. For immigration purposes, you are required to show one year's worth of tuition and costs. You can find the tuition and additional costs of your program on our International Tuition and Costs page.
Make sure you don’t forget these documents. We can’t accept your International Supplement Form without them. Also, please ensure that your financial documents are in English; if they are not available in English, you will need to provide an official word-by-word, line-by-line translation along with the original language.
You will also be asked about the type of visa you currently hold (if any) and the type you plan to hold at Chapman.
How much does Chapman cost?
Chapman's cost varies depending on the program. Each program has its own tuition, plus additional costs. You can view tuition costs per credit for each individual program on our Student Business Services website.
*For your I-20 cost of attendance, you are required to show one year's worth of tuition and additional costs.
What counts as evidence of available funds?
There are four types of funding that you can provide as evidence of your available funds. The type you provide depends on your individual situation.
Personal Funds
You can submit any of the following documents as evidence of personal funds:
- Financial Institution Verification Form that a representative of your bank has completed.
- Bank/financial statement in your name that shows your account balance. This can include multiple accounts. Your bank/financial statement must:
- Indicate the financial institution’s name.
- Indicate the account holder’s name.
- Clearly show the account balance.
- Be dated within the past 6 months (older statements will not be accepted).
- Be translated into English, including names.
- Clearly indicate the type of currency.
- Show liquid assets that can be converted to cash.
Family Funds
Evidence of funds from family must include:
- Your family’s bank/financial statements. These must:
- Indicate the financial institution’s name.
- Indicate the account holder’s name.
- Clearly show the account balance.
- Be dated within the past 6 months (older statements will not be accepted).
- Be translated into English, including names.
- Clearly indicate the type of currency.
- Show liquid assets that can be converted to cash.
- Letter(s) indicating the relationship between you and your sponsor(s), as well as the amount of funding you will be receiving per year. Alternatively, you can submit this Financial Sponsor Information Form.
Funding from Other Sources
Other sources include employers, the government or other organizations. If you get evidence of funding from these sources, make sure it is written on official business letterhead and includes a signature and the amount of funding you will be receiving per year.
We do not accept salary statements as evidence of funding.
Loan Certificates/Letters
Loan certificates/letters must include:
- Your name.
- The amount of the approved loan on official letterhead.
We accept conditional loan approvals only if the condition is the receipt of your I-20.
We do not accept loan applications as evidence of funding.
What happens after I submit my International Supplement Form?
Once you have submitted your International Supplement Form, we will evaluate your documents and issue you an I-20 Form. After you receive your I-20 Form, your next step is to apply for an F-1 visa.
