+-Undergraduate Students

2013-2014 Health Insurance can be purchased through the University and be billed to your student account no later than the end of the third week of each semester.  The annual cost for Health Insurance is $1,434.00 ($717.00 per semester).  Students may request health insurance coverage through the Business Office.  Click on form below to complete and submit your request.

  • To purchase the Health Insurance submit the following form to the Student Business Services Office via email to Ocbusn@chapman.edu.
    The insurance form must be authenticated by emailing it from your Chapman University email account.

**The student health insurance form is an updatable PDF Form: To fill out, key-in the required information use Adobe Acrobat. Mac Preview and other applications will not store information correctly. Tab over each field, or click on each item to update. Print the form, sign it, then email it.

+-Graduate Students

+-Doctor of Physical Therapy Students

All Doctor of Physical Therapy students are automatically billed when enrollment is verified for student health insurance. Cost for the 2013-2014 year is $478.00 per trimester; plus the $81.00 Health Services Fee which allows access to the Health Center on campus.
