Verification Worksheets
If you have completed the FAFSA and you have received a Student Aid Report that indicates you have been selected for verification, you must complete a Verification Worksheet. Please select the type of Verification Worksheet that applies to you and attach a signed copy of your federal taxes, following the instructions on the second page of the worksheet.
After you open the appropriate verification worksheet (.pdf format), you will need to print it out and complete the necessary information. Regardless of which campus you attend please mail your paperwork to: Financial Aid Office, Chapman University, One University Drive, Orange, CA 92866, or fax it to 714-997-6743.
If you think you have circumstances that should be considered when determining your dependency status, you may complete a Dependency Review found on the forms page.

Fill out the Independent form if you meet at least one of the following conditions:
- Born before January 1, 1985
- Veteran of the U.S. Armed Forces or currently on active duty
- Enrolled in a masters or doctorate program during 2008-2009
- Provide 50% or more of a dependent child's support
- Married
- Orphan
- Ward of the court (now or was through age 18)
Fill out the Dependent form if you do not meet any of the conditions above. |
Fill out the Independent form if you meet at least one of the following conditions:
- Born before January 1, 1986
- Veteran of the U.S. Armed Forces or currently on active duty
- Enrolled in a masters or doctorate program at the beginning of 2009-2010
- Provide more than 50% of a dependent child's support
- Provide more than 50% support of a dependent who lives with you
- Married
- Orphan
- Ward of the court (now or was through age 18) or were in foster care after age 13
- Emancipated minor
- In a legal guardianship as determined by a court in your state of legal residence
- Unaccompanied youth who is homeless
Fill out the Dependent form if you do not meet any of the conditions above. |
