»Beneficiary and Survivor Information

When a Chapman University employee dies, or is facing a grave illness, we understand that it is a very difficult time for family members. We would like to support loved ones who may need to make vital decisions, gather important documents or access and complete any required forms. The following information is available to help families navigate these difficult tasks and prepare survivors and beneficiaries with the information they will need to claim benefits following the death of a Chapman employee.

Beneficiary(ies) - Person(s) designated by an employee to receive a life insurance benefit payment or a retirement plan distribution after the death of an employee.

Survivor(s) - Person(s) responsible for the completion and submission of required documentation in order to receive distributions from life insurance policies and retirement plans. If survivor is a spouse, registered domestic partner or dependent child, that individual will be asked to make decisions on continuing benefit coverage through medical, dental and vision providers.

Beneficiary and Survivor Procedures

  • Please notify the university as soon as possible of the death of the employee. If you are unsure whom to call, please contact the Office of Human Resources at (714) 997-6686. At this time, if you are able, you can speak directly to a Benefits Representative who can walk you through the benefits process and provide you with additional resources to help you through this difficult time.
  • A personalized letter will be sent to the family of the deceased employee outlining specific survivor benefits and the procedures required to obtain them. In the meantime, the following contact information is provided for you to speak directly with representatives from our benefit carriers.

We suggest that you have the name of the employee and Social Security Number information available prior to contacting these representatives.

You are encouraged to contact the appropriate representative at your convenience. There is no time frame in which this must be done. However, it is important that you reach out as soon as possible. The representative will be able to provide guidance and documentation as needed.

+-Basic Life and Supplemental Life Insurance

The University provides all full time benefit eligible employees with Basic Life and Basic Accidental Death and Dismemberment (AD&D) insurance. The amount of coverage is two (2) times the employee's annual base salary up to a maximum amount of $800,000.

+-Benefit Plans

Medical, dental and vision coverage will end on the final day of the month in which the employee dies. Coverage can be extended for up to 36 months through COBRA. COBRA is administered by Benefit Concepts and within 10 days of notification the Chapman University Benefits will notify them of the death of the employee. Benefit Concepts will then mail the spouse and/or dependent children COBRA information and provide details on options and cost to continue coverage under this program.

+-Tuition Remission

For questions regarding the Tuition Remission Program please contact a Benefits Representative at (714) 997-6749.

+-Tuition Exchange

The Tuition Exchange benefit shall be provided for the spouse and legal dependents that have been accepted or are enrolled at the time of such death. Beneficiary will continue to receive the Tuition Exchange benefit by maintaining continuous enrollment through the completion of the semester in which they are currently enrolled.
