IMPORTANT INFORMATION REGARDING CHANGES TO STUDENT HEALTH INSURANCE FOR 2012/13
The Health Care Reform Act was approved by the Supreme Court on June 24, 2012. This requires significant changes to individual health insurance.
The Patient Protection and Affordable Care Act (PPACA) signed into law in 2010 established the following parameters of health insurance coverage. For 2012/13 the benefit coverage level must be $100,000.00; for 2013/14 the benefit coverage must be $500,000.00; for 2014/15 the benefit coverage must be unlimited. Note that under this legislation, many students can continue to be covered under their parent's health insurance policy up to the age of 26.
This is a significant change to the Chapman University undergraduate student insurance previously billed as a mandatory fee to student accounts. The university will NO LONGER automatically bill undergraduate student accounts for health insurance coverage. Beginning with the 2012/13 year, health insurance coverage will be an optional service (opt in) that can be purchased through the Student Business Office and charged to the student account.
If an undergraduate student wishes to purchase individual health insurance coverage through the University with a benefit coverage of $100,000.00 they must contact the business office.

PHYSICAL THERAPY STUDENTS
The requirements of your clinical practice affiliation agreement stipulate that the student will provide evidence of health insurance coverage at the beginning of the clinical experience.
WAIVING HEALTH INSURANCE
This coverage may be waived provided you present evidence that you have and maintain throughout the academic year a medical insurance plan providing coverage within the USA that satisfies the minimum requirements shown above. This approval is contingent upon your completion and Chapman University approval of this Application before the last day to add classes. If you waive coverage, Chapman University reserves the right to audit the coverage parameters of your current health plan at any time and to revoke any prior waiver if we determine that the required conditions are not being satisfied.
